Create an issue using a form

Admins, agents, and collaborators in a Service Desk Team role can create issues using a link that goes right to a form. There are two types of links you can use:

  • a general link that allows you to select any service project, request type, and form that you’ve configured on your Jira site

  • a specific link that only allows you to create issues using a pre-selected form and request type, in a specific service project

This allows your team members to quickly create issues on behalf of customers using forms, without needing to navigate to the portal. If desired, to make things even more efficient, you can add these links to your left hand navigation as shortcuts. Learn how to add a shortcut.

You need to be a project admin to access a link to create issues using forms.

To get a general link to create issues using forms:

  1. From the navigation on the left, select Project settings, and then Forms.

  2. Select the form you want to use to create issues using a link.

  3. Select Settings.

  4. Turn on the toggle next to Create new issue.

  5. Select the request type (or request types) you want to be able to create using this form.

  6. Repeat steps 2-5 for any forms you want to use to create issues using a link.

  7. Next to Link, select the link (or copy and paste the URL into a browser tab).

    • You can use this general link to create issues using any form that you’ve configured to Create new issue on your site.

If you’d like to generate a specific link for a specific form and request type:

  1. Go to the general link from the instructions above.

  2. Select a service project, request type, and form.

  3. Use the direct link that appears below your selections to create issues using a pre-selected form and request type, in a specific service project.

Create an issue using a shortcut to a form

To create an issue using a shortcut:

  1. From the navigation on the left, under Shortcuts, find and select the shortcut created by your project admin.

  2. If the option is available, select your desired service project, request type, and form.

  3. Fill out the form, and select Submit.

To create an issue using a form’s direct link:

  1. Go to the link for creating issues from forms. If you don’t have one, ask your project admin.

  2. If the option is available, select your desired service project, request type, and form.

  3. Fill out the form, and select Submit.
