Only Jira and project admins (or users with project admin permissions) can create, edit, and delete forms.
Forms can be created directly from your Jira Work Management project. When you create a form, you can choose the (issue) type for when people submit the form as a work item to your project. This means that you can choose the type of work that your form collects, which will change the type of work item that’s created when that form is submitted to your project.
Each type will have different form fields depending on what kind of information you want to collect.
Your project will only show other (issue) types if you have more than one type for your project.
To create forms for your business project:
Select Forms from the project view navigation.
Select Templates to open to recommended templates drawer, or select Create form to create one from scratch.
Hover over a template to see a preview before selecting. Templates may not be available to all users.
If your business project has more than one (issue) type, you’ll be prompted to select what type you’d like for your form. The fields in the form will differ depending on which type you select.
This means that when someone submits a form, it will be turned into that type of work item.
Drag the fields to be in the order you’d like for your form.
Customize the text in the field by selecting the field label.
Select Preview to see how your form will look.
Your form is now ready to edit, share, or use.
You can only create forms at the standard issue type level, for example a task type. You’re unable to create a form for epics (the hierarchy layer above a standard issue), or subtasks (the hierarchy layer below a standard issue).
To edit a form:
Select Forms from the project view navigation.
Choose the form you’d like to edit.
Customize the title of the form by selecting the title.
Drag the fields to be in the order you’d like for your form.
Select the type at the top-left of your form to choose the type of work that your form collects. This will change the type of work item that’s created when that form is submitted to your project.
Customize the text in the field by selecting a field label.
Select Preview to see a preview of how your form will look.
Your form is now ready to edit, share, or use.

All changes you make to your form are automatically saved for the next time you return.
To delete a form from your business project:
Select Forms from the project view navigation.
Select the of the form you’d like to delete, then Delete form.
Alternatively, you can delete your form by:
Select Forms from the project view navigation.
Choose the form you’d like to delete.
Select the at the top right of the form, then Delete form.
Before you share your form, you can preview it to make sure you’ve captured all the right information.
To preview your form:
Select Forms from the project view navigation.
On the top-right, select Preview to see how your form will look.
