Diagnosis
Both ProForma Full and ProForma Lite are installed on the same instance.
Solution
Uninstall ProForma Lite in case you have both ProForma and ProForma Lite on the same instance and check if the problem is fixed.
If the issue persists, proceed with the next steps:
Execute the following SQL query:

WHERE PLUGIN_KEY = 'com.thinktilt.proforma.lite.add-on'If you get any results, stop Jira, take a database backup, then perform the following SQL delete statement:
