The top-level initiative issue type is automatically created in the instance when a software project of the template top-level planning is being created.
Once the top-level planning project is created, you will notice that the "Top-level initiative" issue type has been added to the issue type section. Additionally, the "Top-level initiative" issue type is mapped to the initiative type above the epic issue type.
