Unable to create a space when Knowledge Base Survey Plugin is enabled

The 'Create' button is disabled/greyed out after typing the space name
The issue occurs with Knowledge Base Survey Plugin version 1.1

There is an xwork name clash with the "ajax" package in the plugin code and this is a bug which is tracked here: CKBSP-37

The bug has been fixed in Knowledge Base Survey Plugin version 1.2. You may upgrade the plugin to fix the problem.
